Application for Pet Rat Adoption For the GA Rat Rescue
The following application allows us to ensure that we match the right rat(s) to you and your family. Please copy the following questions into an email, fill out the application's questions there and send it directly to us at our primary email address [email protected]. Any unanswered or incomplete questions will automatically be returned for further completion. Generally we try to review and respond to each application for approval within three (3) days, but please be patient. ___________________________________________________________________________________ Part I: Personal Name Address Primary Phone Number Are you 18 years of age or older? (Please note you must have your parent/guardian contact us as well) Part 2: Housing What is your housing situation? (Rent, Own my Home, Live with Family, Live with Housemate, Student Housing, Other - please describe) Are there any children /youth living in your household? (List ages) Are you or anyone over 65 years old, immunocompromised individuals and/or pregnant women in the home? Are you or anyone else in your home allergic to rats / rodents / other animals? Part 3: Pet History Have you had rats in the past? Do you currently have rats? (If so, include age, sex and if they are already spayed/neutered) How will you quarantine your new rats from your current rats? How do you plan on introducing your new rats to live with your current rats? What would you do if this animal didn’t get along with your current pets? Do you currently have any other animals living in your household? Part 4: Rat Care Who will be the primary caretaker of the rats in your household? How much time per day can the primary caretaker dedicate to the rats? Where will the rats be kept in the household? What kind of cage setup do you have? (Be as detailed as possible, include dimensions) What type of bedding or litter are you currently using, or planning to use? What type of food/diet are you currently feeding to your rat(s), or planning to feed? Where do you buy your pet supplies? List the name of the veterinarian & veterinarian practice you will be using: Does this veterinarian see rats? Do you need a referral to an experienced rat-knowledgeable veterinarian? What will you do with your rats when you go on vacation? Part 5: What Are You Looking For? The rats I'm adopting will be: (Companions for myself, A gift, Classroom pets, Companion(s) to another rat, Other _________) How many rats are you looking to adopt? *NOTE* While adopting beyond a pair of rats is not required, Rats are very social animals that thrive best in small groups of 3 to 5. Please consider adopting a trio or more or keeping a small group of 3 to 5 rats. Which gender(s) are you looking for? (Females, Males, No Preference) Which age range are you looking for? (Baby/Youth - 6 weeks to 6 months, Adult - 6 months to 2 years, Senior- 2+ years, No Preference) What other traits are you looking for? Please include temperament, colors, markings, and any other preferences you might have. If you have no preferences, please say so Is there a particular rat or case are you most interested in? Would you consider any of the following special needs rats? Typically rats with the traits listed below stay in the rescue under our sanctuary program, but sometimes we have requests for special needs rats, so if you're one of those few, please indicate which: 1) Rats that need special mobility accommodations 2) Rats that need special accommodations and handling due to neurological issues 3) Rats that may have medical issues like chronic respiratory problems, tumors, or physical injuries needing additional care 4) Rats that have suffered previous trauma and are difficult to handle, may scream, or may even bite 5) Neutered male rats that continue to be aggressive toward other rats and need to remain living alone 6) Elderly rats 2+ and older with some age-related health issues Part 6: Final Additional Questions How did you find out about us? I have answered all questions truthfully and understand if I do adopt a rat from the Georgia Rat Rescue, this application becomes a part of my adoption contract. By sending in this adoption profile you agree that the information you have given us is true to the best of your knowledge.*** No signature is required at this time. You may use as much space as you need. VERY IMPORTANT!! While we love young people who love animals, unfortunately we cannot enter into a legal contract with anyone under the age of 18. If you are younger than 18 the adoption profile must be filled out by a parent or guardian. We will need to communicate with the parent or guardian and they will need to sign the adoption contract. As the adult in the home, they will be ultimately responsible for the care of the animals. We reserve the right to deny any adoption at any time. All animals are adopted out as pets only. Any applications for animals desired as feeders or breeders will be denied. If you do not currently have rats of your own, rats will be adopted out in same sex pairs (with the exception of neutered or spayed rats). |
